To configure an incident form, you will need to click onto the settings icon and then click onto the “Incident Form Configuration” icon.
This page will then allow you to add new forms as well as adding new tabs, groups, and questions by using the option within the grey tool menu, as per the screenshot below.
Conditional logic, enabling the showing and hiding of fields based on responses to previous questions, can also be configured in this area.
This can be completed by clicking onto the “Add condition” icon where a pop-up screen will appear, allowing you to enter the condition title, choose the condition status and also which form to assign the condition too. Once these fields have been filled in, you will see a new section just underneath the “Conditional Logic” heading and here is where you can create the conditional logic. Once the relevant sections have been completed, click the green plus icon on the right-hand side in order to add the conditional logic.
You are also now able to edit and preview incident forms by clicking onto the relevant icons as per the screenshot below:
